What You'll Do

In this high-energy role, you'll be the face of our organization in the community, helping individuals navigate health plans, connect with essential services, and create a healthier future. Every day, you'll:

  • Represent the Organization: Attend health fairs, community events, and local meetings to share information about our services.
  • Help People Access the Care They Need: Guide individuals through health plan applications and enrolment processes, connecting them to vital resources.
  • Create Welcoming Spaces: Provide support at ACCESS Centers, assisting visitors, answering questions, and helping operations run smoothly.
  • Track and Report: Meet performance goals, document outreach activities, and ensure accurate follow-ups to improve patient access to care.

What We're Looking For

We want someone who is passionate, compassionate, and ready to help communities gain access to the care they deserve. Here's what you'll need:

Qualifications:

• High School Diploma required; associate's or bachelor's degree in social work, public health, or a related field is a plus.
• 12 years of experience in community outreach, customer service, or healthcare.
• Bilingual? If you speak Spanish or Creole, you'll be an even better fit!
• Strong communication skills and the ability to build relationships quickly.
• Flexible schedule: Able to work evenings, weekends, and travel locally when needed.
• Tech proficiency: Comfortable with Microsoft Office and data entry systems.
